To assess the role of pregnancy-associated plasma protein A (PAPP-A) and free β-human chorionic gonadotropin (β-hCG) for the diagnosis of unexplained bleeding in pregnant women, with special attention to gestational age. In this prospective cohort study, we assessed PAPP-A and free β-hCG in the serum of pregnant women with unexplained vaginal bleeding and/or mild bleeding in the lower uterine segment. Free β-hCG was determined using an electrochemiluminescence method. The results were compared to the results of the first trimester screening for fetal aneuploidy (NT) and from the first and second trimester combined tests (combined test) of Down syndrome. We analyzed a total of 592 women between 11 and 14 weeks of gestation. The prevalence of abnormal PAPP-A and β-hCG values were 13.2% (77/592) and 17.6% (100/592), respectively. In the group of women with “unexplained” bleeding, the prevalence of abnormal PAPP-A values was 14.8% (26/177), and the prevalence of abnormal β-hCG values was 15.4% (26/170).
